BREAST IMPLANTS SHAPE OPTIONS

As stated above, there are two basic breast implant shapes: teardrop and round. Naturally, you will have a long list of other choices to make about your breast implants as well (size, projection, material, etc.), but when it comes to breast shape, you just have these two options:

Teardrop Breast Implants

Teardrop breast implants have an asymmetrical shape, which means there is a “right way” and a “wrong way” for them to be inserted into place. Because teardrop implants must be placed a certain way, this means teardrop implants cannot rotate while inside your body either.

It can be difficult to stop Teardrop implants from rotating, however, so manufacturers have designed teardrop breast implants to have a very slight texture on their surface. This texture helps keep teardrop breast implants from rotating or otherwise shifting position.

Round Breast Implants

Round implants are — as the name implies — perfectly round. If you were to set a round implant flat on a table, it would look the same all the way around and from every angle. Because tear drop and round breast implants are perfectly symmetrical, it doesn’t matter what side goes up or what side goes down when your surgeon places them during surgery.

Furthermore, as you heal and go about your life, round implants can rotate fully without consequence. Your breasts will never look different because it doesn’t matter which end is up.

And finally, because round implants can freely rotate without consequence, no texturization is necessary on their outer surface. In this way, you’ll find that round implants are perfectly smooth and may be slightly safer for this reason.

“Side boob” is a highly sought-after trend that has been getting a lot of attention in recent years.

What is side boob? Basically, it’s the appearance of the side of the breasts, which may be noticed in somewhat revealing clothing — clothing that tends to only cover the front of the breasts. For example, certain tops and dresses may accentuate “side boob” when they don’t cover as much of the side of the trunk as other tops and dresses.

Teardrop shaped breast implants tend to provide better side boob because the lower portions of the breast shape are accentuated more than the tops, and this is what is revealed in the profile view.

They should not move during the healing process.

Teardrop implants will not typically move during the healing process, which is a critical time during your recovery from teardrop breast augmentation. They won’t move because they have a textured surface, which helps adhere to the tissue.

They often work well for petite women.

Teardrop implants can be great for shorter women, especially when there is little breast tissue to work with. That’s because round implants can be difficult to fully cover with only a minimal amount of breast tissue.

More petite women tend to have only a small amount of extra breast tissue on their chests, so this causes what we refer to as constricted lower pole. In other words, the skin becomes too taut over the new implants on the lower pole. Round implants are preferable in this situation.

They can be good for taller women too.

Taller women have longer torsos. To match the anatomy of a woman with a longer torso, teardrop shaped breast implants are often used for breast augmentation.

That’s because, unlike round implants, teardrop implants tend to be longer than they are wide (round implants are the same diameter all the way around; they are a perfect circle). In this way, teardrop shaped implants match the anatomy of taller women better.

They can often help women with slightly sagging breasts.

It’s not uncommon for patients to want both bigger breasts and lifted breasts. However, the surgeries necessary for fixing these issues are totally different.

Breast implants surgery (also known as breast augmentation surgery) involves increasing the size of the breasts with implants.

On the other hand, a breast lift aims to simply lift the breasts higher on the chest wall. This surgery is for women who have droopy breasts that tend to hang down lower than they used to. When a woman has breast lift surgery, she has enough tissue (in other words, the size of her breasts is not the issue), but the skin has been over-stretched.

One great thing about teardrop shaped implants is that they can actually help the patient who wants a breast lift and breast augmentation to avoid the need for a breast lift (while still augmenting the breast size). This is because teardrop shaped implants help to actually raise the lower portion of the breast — effectively tightening that area and adding a bit of lift while also increasing the overall size of the breasts.

DRAWBACKS OF TEARDROP BREAST AUGMENTATION

Teardrop shaped implants aren’t right for everyone. Here are some drawbacks to be aware of when it comes to this breast implant type for breast augmentation.

They can rotate.

Likely, the biggest drawback of teardrop implants shaped implants is that they can actually rotate after they are placed. With round breast implants, this isn’t a problem because round implants have the same diameter and projection all the way around; they are symmetrical.

Unfortunately, if a teardrop shaped implant rotates, this can cause a serious anatomical problem for the patient. For example, if a teardrop implants were to rotate about 180 degrees, this would cause the upper portion of the breast to appear much larger than the bottom portion. This would result in a noticeably unnatural appearance in your breast augmentation.

They can look unnatural when you lie down.

When a woman without breast implants lies down on her back, the breasts simply splay out in a natural way — the same on all sides. Round implants will have this same effect.

However, a teardrop shaped implant has a permanently fuller lower pole. Therefore, if a woman with teardrop shaped implants were to lay down on her back, the breasts would not lay quite the same way, and this would result in an unnatural appearance.

Leaks can be difficult to detect.

Normally, teardrop shaped breast implants are made of silicone. While silicone has many benefits, one drawback is that patients must get regular MRIs or sonograms to ensure that a leak has not occurred. This is because leaks are difficult to detect with silicone implants.

Unfortunately, getting an MRI for plastic surgery every several years when you have teardrop implants can be rather burdensome.

They’re not ideal if you already have a good breast shape. 

Remember that breast augmentation is a surgery performed to increase the size of the breasts. Sometimes, an implant may actually replace the entire breast (as is the case with reconstructive surgery after breast cancer and mastectomy, for example). However, much of the time, the patient has at least a little bit of breast tissue to start with.

When this is the case and the patient already has a decent breast shape — with more fullness on the lower pole — adding a teardrop shaped breast implant (which also has more fullness on the lower pole) is not going to create a good final shape. Instead, it’s going to add an unnatural amount of fullness to the lower pole and will create an unnatural looking breast altogether.

Increased risk of developing BIA-ALCL

This is one of the most common drawbacks you’ll hear about in relation to teardrop implants.

BIA-ALCL in plastic surgery stands for Breast Implant Associated Lymphoma. This is a form of immune system cancer — a type of non-Hodgkin’s lymphoma. The theory is that because teardrop shaped breast implants are textured, the breast tissue inside the breast actually grows into the texturing and can possible lead to BIA-ALCL.

I’ve Had My Breast Implants for 20 Years: Do I Need a Replacement?

Every so often, a patient will come in and say, “I’ve had my breast implants for 20 years, and now I’m wondering if I need to get them replaced.” Sometimes, patients will even say, “I’ve had my breast implants for 30 years,” and they want to know if that is “too long”.
In fact, these concerns are not completely unfounded. Despite the durability of most modern breast implants, the devices are not meant to last an entire lifetime.

At some point, breast implant removal or replacement will be necessary.

How long you personally will be able to safely keep your breast implants depends on multiple factors, which we will discuss below. First, however, let’s look at the potential breast implant pros and cons.

Can Old Breast Implants Cause Problems?

Do you have to replace breast implantsYes, it is possible for old breast implants to cause problems such as capsular contracture, ruptures, deflation, and in rare cases, breast implant illness (BII) or a specific cancer called breast implant-associated large cell lymphoma (BIA-ALCL). However, these issues are not inherent in all old implants. 

It’s really up to you to follow your plastic surgeon’s directions for breast implant care. Continue monitoring the state of your implants and the state of your overall health so that you can determine the best time for replacement or removal.

What Happens if You Don’t Replace Breast Implants?

For some, replacement of implants after 10 or 20 years will not be necessary, and the implants will continue to do what they were meant to do without issue. As long as your implants continue to fit your lifestyle and you are not having health issues related to them, there is generally no reason to have them removed. 

With that said, after several decades or longer, it becomes unlikely that breast implants can stand the test of time. Eventually, most implants will fail by way of rupture, deflation, or other reasons. After this point, it’s important to have your implants removed or replaced for your own safety.

Should Breast Implants Be Removed After 20 Years?

This really depends. If, after 20 years, your implants are in good shape, you are in good overall health, and you are happy with the way your breasts look, there is really no reason to have your implants removed. However, do remember that your risk of rupture increases over time, so it’s important to continue monitoring your implants so that you can catch any ruptures or leaks before they cause problems.

Rippling or wrinkles

Aesthetically, ripples and wrinkles can sometimes cause problems for patients. These surface issues are usually only noticed by touch, but sometimes, they are visible as well.

Rupture of silicone implants

When silicone implants rupture, it can be difficult to tell. Patients usually feel a burning or tingling sensation and will notice the size of their breasts appears uneven. Regular checks of silicone implants can help catch “silent ruptures”.

Rupture of saline implants

When saline implants rupture, it is instantly noticeable because the implant itself will literally deflate, and your body will absorb the leaked saline. 

Shifting implants

Implants may change position over time and end up looking uneven or off-center. This usually happens because of an extreme change in weight or because of aging. Some patients will opt to keep their implants and get a breast lift if this issue occurs.

Capsular contracture

After getting implants, scar tissue will naturally form around each device. Unfortunately, this tissue sometimes forms in a way that causes discomfort, tightness, and noticeable hardness on the breasts. The breasts may even appear misshapen.

Personal decision of patient

Finally, some patients simply decide that implants no longer fit their lifestyle. Body shapes and aesthetic preferences can change over time, and removal is an option when this happens.

Commonly Asked Questions About Breast Implant Replacement

What do breast implants look like when you’re old?

Aside from issues such as rupturing, capsular contracture, or implant related illnesses, in some women, implants can cause the breasts to sag more because of their weight. Sometimes, this is called “bottoming out”. Other issues may include implant displacement (when implants shift out of place) and capsular contracture (the overproduction of hard scar tissue around your implants).

Can your body reject breast implants years later?

Your body will not “reject” your implants. The term “rejection” is used for live tissue that is transplanted into someone’s body — as in the case of a liver transplant. In this case, the liver may be rejected by the body. While your body may not tolerate breast implants well — now, or after several years — your body will not “reject” them.

What happens if breast implant is left in too long?

If breast implants are left in too long and rupture or cause problems such as capsular contracture, breast implant illness, or something else, this will greatly affect your health. 

In terms of immediate problems, as soon as an implant leaks or ruptures, you need to see a surgeon and have it removed for your safety. In most cases, replacement of the ruptured implant will be an option.

Can silicone breast implants last 30 years?

In rare cases, silicone breast implants may last for 30 years. However, most patients will decide to undergo replacement or removal surgery before this time.

The lifespan of your implants can potentially be extended indefinitely. In actuality, there is no predetermined time frame for replacing breast implants. Saline or silicone implants typically have a lifespan of approximately 10 to 20 years, but they can endure beyond that. Can reduction surgery correct breasts that are asymmetrical ?

Yes, some women decide to undergo breast reduction surgery to reduce the size of larger breast and make it more like the other. Alternatively, patients have the option of undergoing breast augmentation surgery (or breast fat transfer augmentation) to increase the size of the smaller breast.

Does insurance cover breast asymmetry surgery?

Whether or not insurance covers breast asymmetry surgery really depends on a multitude of factors. If fixing your asymmetry is deemed medically necessary, your insurance provider may cover all or part of the procedure. This isn’t always the case, however, so always check with your provider before assuming your surgery will be covered.

7 Things to Expect After a Tummy Tuck

If you’re considering a tummy tuck, get ready to embrace a brand new body and brace yourself for the worst days after Tummy Tuck surgery. The recovery process can be intense and challenging, lasting several weeks. However, having a few essential pieces of information at your fingertips can make the healing period more manageable.

Here are seven things to expect after a tummy tuck:

1. YOU WILL EXPERIENCE SOME PAIN AFTER TUMMY TUCK, BUT THERE ARE WAYS TO MANAGE IT

Any invasive surgery comes with some pain, and tummy tucks are no exception. The surgery requires removing both fat and skin, and usually suturing the abdomen’s connective tissues as well, so that they hold together more tightly. All of this will cause pain, tightness and lack of mobility during the Tummy Tuck recovery period.

One option that your surgeon may offer you is a non-narcotic, non-opioid pain reliever called Exparel which is injected directly into the abdominal muscles, tissue and fascia during surgery. This long acting medication can reduce post operative pain for up to 72 hours. Exparel will add about $500 to the cost of the surgery, but the significant reduction in pain and increased comfort is well worth it for most patients.

2. YOU MUST WEAR YOUR COMPRESSION GARMENT

Some physicians will even refuse to do the surgery if they don’t feel you’re taking the smoking ban seriously, so wrap this into your pre-op preparations today.

7. YOU WILL NEED SUPPORT

Many people assume that they can “get around fine” after the first day or two of the surgery, but that’s not the case. Not only is a tummy tuck a major surgery, but you may not realize how much “heavy lifting” of daily movement that your abdominal muscles do. Once you have an incision in this area, every normal activity becomes more difficult. Movements such as reaching up high, sitting up, and even a cough or sneeze may be very uncomfortable indeed for several weeks.

That’s not to say you will need help during your entire recovery, but at a minimum, you should definitely arrange to have someone helping you for the first week after your surgery. You will need to help with child and/or pet care, making meals, getting in and out of bed, moving around your home, dressing and bathing. After about two weeks, you’ll be mostly back to your regular routine and able to finish your normal daily to-dos (except for exercise- you will need to wait 6 weeks to begin exercising again).
